Collect supporting documents

  • Copy of valid KTP/identity card or copy of birth certificate if insured is underage

  • A second original medical report from the attending physician stating that the insured remains permanently disabled after a waiting period of 6 months from the diagnosis

  • Original medical report from the attending physician stating that the insured is permanently disabled caused by illness or by an accident.

Submission Timeline

  1. Maximum of 60 days from treatment to file a claim.

Receive e-mail Notification

  • After claim is processed, HRD will receive an e-mail notification from FWD Life.
  • For an incomplete claim, the insured has 30 days to submit the missing documents.
