It’s always a good idea to first check that your policy covers your claim. The quickest way to do this is via customer eServices on FWD MAX. Through eServices, you can submit a claim as well as check the status of any claims that are active. You can also get in touch with us directly (go to Support).
Access eServices by downloading the FWD MAX app, totally free.
Each insurance and sharia plan has different benefits, and therefore has its own claim form. So the first step is to select your type of claim.
If there’s been a death, please select 'loss of life'. If not, please select the living benefit you’re claiming for. Living benefit covers everything apart from a loss of life (for example a critical illness, hospitalisation, or a disability).
If you’re an FWD insurance card holder, please remember that this entitles you to cashless medical treatment within our network of hospitals. Please remember to always keep your card with you in case you need it.
You can see your claims status on the eServices menu via our FWD MAX app. You can also check by contacting our customer services team on 1500525 or by emailing CustomerCare.id@fwd.com.
We aim to complete the process within 14 working days (assuming we have all your documents). If any documents are missing, we’ll email to let you know.
For faster claims, you can use eServices available on the FWD MAX app. Your claim will then be processed within 5 working days. If you submit via eServices, you can also check the status of your claim, using the app. If you use eServices, we’ll notify you via the app to let you know if any documents are missing.
If you don’t submit a claim via eServices, you can still check the claim’s status by calling our customer services team on 1500525.
