If you wish to do this, you need to fill out our non-financial change form. Please include a photocopy of your valid identity card (ID) and photocopies of other supporting documents (birth certificate, marriage license certificate, family card) and any medical records.

- Transaction statements (we’ll send these following any transactions you make such as withdrawal of funds, premium payments, and fund switching). We’ll send a transaction statement on each payment due date.
