Collect supporting documents

  • Copy of valid KTP/identity card or copy of birth certificate if insured is underage

  • Original report from the police or a certified copy issued by an authorized institution if death is caused by a traffic accident or by unnatural causes

  • Original death certificate or certified copy from authorized institution

  • Original police report or certified copy issued by authorized institution if death is caused by traffic accident or unnatural causes

  • Letter of inheritance and documents to prove the relationship with the insured (family card/marriage certificate/ birth certificate), and beneficiary’s ID if insured requests payment to be made to the beneficiary

Submission Timeline

  1. Maximum 60 days from the date of death to file a claim.

Receive e-mail Notification

  • After claim is processed, HRD will receive an e-mail notification from FWD Life.
  • For an incomplete claim, the insured has 30 days to submit the missing documents.
